⚠️Warning

  • Failing to enroll results in backdated premium billing — you may owe months of unpaid premiums.
  • Health insurance cards must be renewed annually.

📋Step-by-Step Guide

1

Enroll in National Health Insurance (国民健康保険)

Visit your local city hall within 14 days of arrival or after losing company insurance. You'll need your residence card, passport, and My Number.

💡 Tip: If you have company-sponsored health insurance (社会保険), you don't need to enroll in National Health Insurance.
2

Apply for premium exemption if applicable

Low-income residents can apply for partial or full exemption of National Pension premiums. Apply at your city hall with proof of income (or lack thereof).

3

Enroll in National Pension (国民年金)

Most foreign residents working in Japan are required to contribute to the pension system. Your employer handles this if you have company insurance.

4

Receive your Health Insurance Card (保険証)

Your health insurance card is mailed within 1-2 weeks. Present it at hospitals to receive the standard 30% copay rate.

5

Track your pension contributions (年金記録)

Use "ねんきんネット" (Nenkin Net) online to check your pension records and estimate your future benefits.

💡Helpful Tips

  • Lump-sum pension withdrawal is available when leaving Japan permanently — you can claim back contributions.
  • The health insurance copay is typically 30% for adults (70% covered by insurance).
  • If you lose your job, apply for the special reduced premium rate immediately.
